For more than a decade, Spectrum Health has successfully led a remote patient- monitoring program focused on chronic-disease patients with the primary diagnosis of heart failure, COPD and diabetes. By leveraging technology, daily monitoring of key health metrics and behavioral coaching this program effectively reduces costly hospital readmission's and can significantly increase a patient’s quality of life.

Remote patient monitoring

Remote patient monitoring is the process of using remote monitoring devices to help monitor your residents and significantly reduce re-hospitalization and emergency room visits. Interactive voice response systems are used for daily check-ins to monitor signs and symptoms of chronic disease or acute illness to quickly catch any worsening medical issues. A nurse will review your data, connect with you and your physician to come up with a plan to help you avoid hospitalization and stay well in your home. Monitoring devices can track blood pressure, heart rate, weight, blood glucose, and oxygen saturation levels.

Visiting Nurse Association is now offering virtual therapy in addition to in-person therapy sessions. Virtual therapy touch points may include the use of a smart device application, face to face virtual visits, or check-in phone calls to ensure you are progressing on your rehabilitation journey. Early outcomes of adding virtual therapy as part of home care have demonstrated that patients are meeting their goals more quickly and with more frequency than without.

On demand video visits on MyChart use technology to bridge the distance between your residents and a health care provider. The consultation is similar to a typical office visit, but uses a secure video connection. It is available wherever your residents have Wi-Fi access. All interactions are secure and HIPAA compliant to ensure confidentiality.

Video visits can take place via webcam anywhere a strong Internet connection exists. Such visits provide direct, real-time access to a primary care provider for consultation in regards to such non-life-threatening conditions as:
