Community Health Needs Assessment

Spectrum Health hospitals have developed Implementation Plans dedicated to "improving health care access and improving health literacy, awareness and education" as identified in the "Community Health Needs Assessments" conducted in each county throughout the Spectrum Health service area.

Spectrum Health collaborated with numerous partners including area hospitals, mental health providers, community agencies and foundations in the collection of data for the health needs assessment.

Spectrum Health hospitals in the counties of Barry, Kent, Lake, Mason, Mecosta, Montcalm, Newaygo, Northern Oceana, Osceola, and Ottawa collaborated with their regional Community Health Needs Assessments and developed dedicated Implementation Plans.

The focus of this Community Health Needs Assessment is to identify the community needs as they exist during the assessment period (late 2014-early 2015), understanding fully that they will be continually changing in the months and years to come. For purposes of this assessment, “community” is defined as the county in which the hospital facility is located. This definition of community based upon county lines, is similar to the market definition of Primary Service Area (PSA). The target population of the assessment reflects an overall representation of the community served by this hospital facility.

The information contained in this report is current as of the date of the CHNA, with updates to the assessment anticipated every three (3) years in accordance with the Patient Protection and Affordable Care Act and Internal Revenue Code 501(r). This CHNA report complies with the requirements of the Internal Revenue Code 501(r) regulations either implicitly or explicitly.
